An assistant practitioner who is dedicated and passionate is required to join our Pathway 3 service, which is situated in Maidstone. Pathway 3 is a component of the Home First/DDischarge to Assess Process. We facilitate the transition of individuals to a local care home for whom an additional period of assessment is necessary to establish a long-term care plan after an acute hospital admission is no longer necessary.

We collaborate with the Discharge Liaison Teams of acute trusts, the Social Services Teams of Kent County Council, All Age Continuing Health Care, local community services, and organizations, including the voluntary sector. We collaborate to facilitate the assessment of individuals in a timely manner to ascertain their long-term needs.

Duties of Assistant Practitioner Jobs in UK

In this demanding position, you will be responsible for triaging all referrals and determining whether they are appropriate for the service.

Prior to hospital discharge, you will be in contact with referrers to request any supplementary documentation. You will be responsible for uploading documents to the RIO under the Community Services System. While RIO would be advantageous, training will be provided.

As you will be navigating a complex caseload document, proficiency in Excel would be advantageous. Additionally, it will be necessary for you to exhibit exceptional interpersonal skills, as you will be responsible for addressing numerous inquiries and engaging in conversations with patients, their relatives, and caregivers.

Additional Working Pattern: 9.5 hours per day, distributed over four days, with a fixed working day to be confirmed, as well as one rotational Saturday per four.

Evidence of Foundation Degree Level Study (L5) or equivalent demonstrable experience
